She controls it with diet, which is damn near impossible with Type 1, which she has. I would not be surprised if she starts loosing her feet soon.
Diabetic woman can and do have children, but usually they have to be much younger than 39.
Halle does take insulin injections and has done PSAs for diabetes awareness. Yes, she is strict with her diet, but it's not the only method she employs to monitor/control her diabetes.

As well as other photos of them from the same week and earlier in the month/last month.
Looks like a false alarm, huh? More new photos of Halle on splashnews.com of her wearing an open white top w/orange bikini top and beige khaki shorts and a very flat stomach running from the papps. No visible baby bump.
