She is absolutely not a 42 DD - the 42 refers to the circumferance of the ribcage. A 42" ribcage means you're either pretty overweight or naturally built like a linebacker. If she had a 21 inch waist - that would mean a 21 inch difference between her waist and ribcage. I'd say she's a DD, but probably 32 or 34DD.
